Setting an Appointment
To request an appointment at Parker Sports Medicine & Orthopedics, you can contact us at (806) 350-2663 and have one of our admitting personnel assist you.
To facilitate the appointment process, please have your insurance card available as we will ask you about your coverage. If your insurance plans have office visit co-pay, you will be expected to pay the co-pay at the time of your visit.
Getting Ready for Your Visit
We require that you bring any recent X-Rays, MRIs or CT scans with you to your appointment. You will have to contact that facility where the procedures were performed and sign a release for the films. We would like to have the film and reports. Without those films, we may have to reschedule your appointment so that we do not duplicate diagnostic tests. In some cases, the films or techniques are not adequate and may need to be repeated.
You may come to the office approximately 30 minutes before your appointment time and complete requisite paperwork or you can download the forms below, complete them, and bring them with you at the time of your appointment. You can also complete your registration online through our Patient Portal.
