Insurance Filings
You, as the responsible party, must furnish our office with the up-to-date insurance information. A copy of your insurance card will be made. Report any changes in insurance coverage immediately upon arrival. Any balance due, not covered by insurance, is your responsibility and is due upon receipt of statement from our office.
Self-Pay Patients or No-Insurance
If you have no insurance and/or have experienced hardship, PSM has developed an affordable policy in which you can utilize:An office visit in which an x-ray is needed is $250.
- An office visit in which an x-ray is needed is $250.
- An office visit in which an x-ray is not needed is $200.
- There may be additional charges after being seen by your physician. These charges will be provided to you upon check out and payment plans are available. These additional charges could include casts/splints/injections/additional x-rays/procedures.
- If you need to set up a payment plan, please call the billing office to discuss or set it up at your appointment in person.
Past due accounts
For accounts not paid in a responsible manner, we are now sending them to a collection agency which will affect their credit, along with their account being blocked for future services until the balance is cleared.
